PROBLEM

Client providers contacted patients via phone to answer long surveys before prescription shipment and for other programs. The process is time consuming for both provider and patient and subject to delays due to multiple attempts to call. Client wanted a digital solution that converted long surveys (up to 80 questions) into easy to complete mobile friendly forms with drop down answer menus and other simple features.

GOAL / PURPOSE

Improve patient experience by allowing patients to complete forms in their own timing wherever they are, increase business efficiencies for high cost provider resources and sent prescriptions without delays.
